BOND with Nature
o Find a peaceful place in the park to reflect. How can you appreciate the natural beauty and draw energy from this connection?
FUEL yourself with Nature
o Think about all the ways we take in Nature – what do we see, hear, smell, taste, feel?
MOVE in Nature
o We can move in Nature in lots of ways – walk on a trail, swim in the lake, swing in a hammock, breathe the air, etc. How do you best like to move?
REST in Nature
o Let your mind take a break from the stimulation of the town/city, technology, lights, etc. Sit for 10 minutes and bring awareness to Nature.
GIVE back to Nature
o Opportunities can include volunteerism, or other simple things like picking up litter or gently taking down stacked river rocks to improve habitats for creek critters. How will you give?
We teamed up with Mount Rogers Community Services, and Hungry Mother State Park, to pilot our 5 Bridges Wellness In Nature immersive project, Forest Fridays. This 5-week program took participants out of the offices and into nature for restorative time each week, grounded in our 5 Bridges system. It was a great success with participants spending a collective 260+ hours in the park to-date.

ABOUT US
5 Bridges is holistic and practical wellness presented in an easy-to-adopt system. Created by Jacob Moore, Founder of NoStigmas mental health nonprofit, this evidence-informed system is the result of two decades of research, clinical opinion, lived expertise, and experimentation. From inpatient and outpatient facilities, private and public schools, for-profit and non-profit organizations, we empower staff and clients to build resilience and learn to thrive in life. More than 10,000 individuals have been served to date—with over 5,000 in Virginia Community Service Agencies alone.
What is 5 Bridges?

Who can benefit from 5 Bridges?Anyone within your service area can use 5 Bridges— clients, staff, community, and even for billable hours. 5 Bridges is currently being used for youth programs, employee wellness, mental health clinic patients, substance use rehab, post-incarceration support, law enforcement and first responders, suicide and substance use prevention, community education, paired with crisis intervention programs, and for anyone who could use support in their wellness journey!

What tools, resources, and support are available?We'll provide you with a step-by-step roadmap for implementing the 5 Bridges program. This 3-part process prepares your agency, coaches and community for success with the system. Additionally, you'll receive our full curriculum, guides, and supplemental materials. Do you provide usage data or success metrics?Yes. On a quarterly basis, we report the total number of users from your group, how many of your group's codes have been redeemed, location data, number of sessions, and more. We combine this with the data you report to us for your grant and compliance purposes.

Will you help us with marketing & distribution?Absolutely. We can provide live talks, hosted webinars, tabling materials, cobranded social graphics, written email copy, and more. Basically, we do the "heavy lifting" so that you can focus on doing what you do best— serving your community! How can we fund the program?Agency Partners primarily utilize grant funding for our program. Federal, state, local, coalition and private grants have all successfully been used. We can help by providing proven-effective grant templates, logic models, lessons learned, and case studies to help you sustainably fund 5 Bridges. What about HIPPA?We never ask you to share your client information with us. All site usage reported to you from your community is anonymous. Since we don't collect any medical information, we are not obligated to comply with HIPAA regulations. Despite this, we voluntarily submit to audits and reviews just to make sure we’re doing our part.

Integrative wellness planning is tailored to the individual's needs, preferences, and goals. This personalization helps increase motivation and commitment to therapies, treatment plans, reentry strategies and additional health and lifestyle goals.
INTEGRATIVE APPROACH
The holistic nature of integrative wellness planning ensures all aspects of an individual's life are considered in treatment planning. This includes physical health, mental health, social connections, environment, lifestyle habits, and more. By examining these elements in relation to one another, it becomes easier to identify and address potential barriers to treatment adherence.
EDUCATION & SELF-EFFICACY
Our integrative wellness planning includes educating individuals about their wellness challenges and the importance of adhering to their treatment plans. This can enhance self-efficacy and empower individuals to actively engage in their self-improvement process.
PREVENTATIVE & COORDINATED CARE
Integrative wellness also involves preventive measures. It encourages lifestyle changes and healthier habits that can help manage symptoms and reduce risk, while increasing resilience and program adherence. We encourage the involvement of a team of professionals from various disciplines. This level of coordination can help ensure that potential roadblocks are identified and addressed promptly.
Regular check-ins or follow-ups can help keep individuals accountable for their treatment adherence. This often takes the form of coaching or peer support. This shared experience can help individuals feel less isolated and more understood, which can improve their motivation to stick with their plans.

HOW YOU CONNECT
BOND is the act of connecting to anyone or anything that provides you with some form of support. This can include your beliefs, people, pets, places, objects and even knowledge—your sources of strength.
FUEL is anything and everything that goes into your system in any way. Beyond food, we look at conversations, media, and even the air we breathe. Everything that goes in has an impact on the system.
WHAT YOU INTAKE
MOVE is the kinesthetic output of your body. Most commonly, we think of physical fitness, but this can also include the internal movements that happen within your body all the time, like circulation & digestion.
WHAT YOU DO
WHEN YOU RECOVER
REST is any time your body, mind or spirit get to recover and reset. Sleep is our primary source of REST, but we go deeper to examine other approaches, such as time in nature and restorative practices.
GIVE explores how we contribute to others and the world around us in positive ways. GIVE can elevate us to another level and feed our soul, but as with every Bridge, it's important to learn balance and boundaries.
YOUR OUTPUT
5 Bridges is person-centered integrative wellness planning that is accessible for most adult and youth populations, including those with risk-factors related to trauma, mental illness, substance use, incarceration, and chronic illness. Evidence-based practices are the foundation of our curriculum and coaching process. We follow these approaches: - Trauma and Grief-Informed - Harm-Reduction Model - Motivational Interviewing (MI) - Health Belief Model (HBM) - Theory of Planned Behavior (TPB) Agency staff and wellness professionals have also found success utilizing the 5 Bridges method. Addressing challenges like work-life balance, stress, burnout, and compassion fatigue allow those who serve to also feel supported. APPROACH
Our methodology is person-centered and seeks to meet program participants where they are with non-shaming approaches that help to increase capacity and build resilience, supporting treatment objectives and overall wellness.
